PEG-MGF (PEGylated mechano growth factor): what it is, what is claimed and what has been studied

PEG-MGF is mechano growth factor, a synthetic 24-amino-acid peptide from the E domain of an IGF-1 splice variant, with a polyethylene glycol chain attached to slow its clearance from the body. It is sold for muscle growth and recovery, but we could not find a single published study of the PEGylated peptide in cells, animals or people. Everything known about it is inferred from the unmodified MGF peptide, whose own evidence is contradictory, and from what PEGylation does to other proteins.

What is PEG-MGF?

To understand PEG-MGF you need two pieces. The first is MGF, or mechano growth factor. The IGF-1 gene can be spliced in more than one way, and the variant that Goldspink’s group in London found switched on in loaded and damaged muscle was named mechano growth factor; in humans it is IGF-1Ec [1] [2]. The peptide sold as MGF is a synthetic 24-amino-acid fragment of that variant’s E domain, the tail that is normally cut off from the pro-protein during processing [3]. The second piece is PEGylation. Polyethylene glycol, or PEG, is a water-soluble polymer that can be chemically attached to a protein or peptide. PEGylated drugs stay in the body longer, are broken down more slowly by enzymes, and are often less likely to provoke an immune response, which is why PEGylation has been used in a number of licensed medicines [4]. PEG-MGF is the MGF peptide with such a chain attached. The intention is to turn a peptide that would otherwise be cleared within minutes into one that lasts days.

That is the theory. In practice, PEG-MGF exists in the scientific literature only as a product name. A PubMed search for PEGylated mechano growth factor, PEG-MGF and related terms returned no primary study of the conjugate; the only hit is a 2026 clinical review that lists PEG-MGF among IGF-1 analogues sold online as research compounds [5]. How PEG-MGF is thought to work

The proposed mechanism is borrowed entirely from MGF. Goldspink’s model is that after loading or injury, muscle first expresses the MGF variant, whose E domain activates satellite cells, the muscle’s stem cells, and makes them proliferate before the IGF-1Ea variant drives them to fuse into the fibre and repair it [1] [2]. The 24-amino-acid E domain peptide increased myoblast proliferation and held back differentiation in a 2002 cell study, apparently through a receptor other than the IGF-1 receptor [3], and it activated human muscle progenitor cells from young but not old donors in culture [6].

The role of PEG in this picture is to keep the peptide in circulation long enough to reach muscle after a subcutaneous injection, rather than being cleared before it gets there. Sellers therefore describe PEG-MGF as a systemic version to be injected anywhere, and unmodified MGF as a local version to be injected into a trained muscle. Neither claim about distribution has been tested. There is no published measurement of where injected MGF or PEG-MGF goes in any animal.

PEGylation can also change how a peptide works, not just how long it lasts. A PEG chain adds bulk that can reduce binding to receptors, and PEGylated proteins sometimes need higher concentrations to achieve the same effect as the parent molecule [4]. For a peptide whose receptor is unknown and disputed, the effect of adding PEG cannot be predicted. A 2012 study found that the MGF E peptides act by increasing signalling through the IGF-1 receptor rather than through a receptor of their own [7], and if that is right, a PEGylated version would inherit both the effects and the concerns of IGF-1 signalling.

PEG-MGF benefits: what the research shows

Because there are no studies of PEG-MGF itself, any claimed PEG-MGF benefits must be judged on the MGF literature. That literature is split. In favour: the E domain peptide increased proliferation of mouse myoblasts [3], protected hippocampal neurons in gerbils with transient brain ischaemia [8], and preserved heart function and reduced the area of compromised muscle by 35% in sheep with induced heart attacks [9].

Against: in 2014, scientists at two pharmaceutical companies tried to reproduce the effects of MGF peptides on muscle cells and could not. Concentrations up to 500 ng/mL did not increase proliferation of mouse or human myoblasts, did not inhibit their differentiation, had no effect on primary mouse muscle stem cells, and did not activate the reported signalling pathway in cardiac cells, although mature IGF-1 and the full-length IGF-1Eb pro-protein worked in every assay. They also tested a stabilised version of the peptide with the same negative result. The authors questioned whether MGF has any physiological role [10]. A 2010 review had already noted that no natural MGF peptide has ever been isolated from tissue or body fluids, so the synthetic peptide may not correspond to anything the body makes [11].

The one animal study that comes closest to the bodybuilding use is discouraging. When the rodent equivalent of the MGF E peptide was expressed in mouse muscle from a viral vector, so that the muscle was exposed to it continuously, the muscle grew larger but became weaker [12]. Sustained exposure is exactly what PEGylation is designed to produce. No study has tested whether PEG-MGF or MGF builds muscle, improves recovery or changes body composition in a trained animal or a person.

Human studies and clinical trials

There are no human studies of PEG-MGF, no case reports and no registered trials. The human research on MGF measured the body’s own MGF messenger RNA in muscle biopsies: it rose after heavy resistance exercise in young men but not elderly men [13], rose in elderly men given growth hormone with training [14], and did not change in healthy young men given two weeks of growth hormone despite a threefold rise in blood IGF-1 [15]. These findings are about the muscle’s own transcript, not about an injected peptide, and they cannot be extended to PEG-MGF.

For a sense of what PEGylation does to an IGF-1 pathway molecule in people, the closest published example is a different compound: a PEGylated recombinant human IGF-1 developed by Roche and tested in 62 healthy volunteers in a first-in-man study. PEGylation extended the half-life to 140 to 200 hours, exposure rose with dose, no hypoglycaemia occurred, and the PEGylated form suppressed growth hormone far less than unmodified IGF-1 did [16]. That study concerned the full 70-amino-acid IGF-1 hormone attached to a defined PEG under pharmaceutical conditions. It shows that PEGylation of IGF-1 family molecules can extend their action by days, and it shows nothing about PEG-MGF, which is a different peptide with an undisclosed PEG.

The only other human-related data are from anti-doping laboratories, which have characterised the products rather than their effects. Black-market MGF has contained a C-terminally amidated analogue [17], a modified full-length protein [18] and an R23H variant [19]. We found no published analysis of a product sold as PEG-MGF, so the size of the PEG chain, the site of attachment and even the presence of the peptide in a given vial are unverified.

PEG-MGF half-life and how long it takes to work

Vendors commonly state a PEG-MGF half-life of 48 to 72 hours, against minutes for MGF. Neither figure has been measured. The unmodified MGF peptide has no published pharmacokinetics in any species, and PEG-MGF has none either. The general finding that PEGylation prolongs residence in the body [4], and the 140 to 200 hour half-life of the unrelated PEGylated IGF-1 [16], are the only data behind the claim, and they do not transfer to a peptide of unknown PEG size.

So how long does PEG-MGF take to work? There is no answer. In animals, the E domain effects were measured within days in acute injury models [8] [9], and the muscle hypertrophy in mice developed over weeks of continuous viral expression [12]. None of this involved an injected PEGylated peptide.

PEG-MGF dosage used in published research

No PEG-MGF dosage has been used in any published study, so there is nothing to report. For MGF, published doses are from cells and animals: nanogram-per-millilitre concentrations in culture, up to 500 ng/mL in the negative replication study [10]; 200 nM of the E domain delivered to the sheep heart [9]; and viral gene delivery in mice rather than peptide injection [12]. None of these are doses of PEG-MGF, and none were given under the skin.

The PEG-MGF dosage protocols found online, typically 200 to 400 µg injected once or twice a week on non-training days, are not drawn from any study. The weekly schedule reflects the assumed long half-life, which is itself unmeasured. For the arithmetic of mixing a vial, a 2 mg vial in 1 mL of bacteriostatic water gives 2 mg per mL, so each unit on a U-100 insulin syringe holds 20 µg; the peptide calculator and the PEG-MGF calculator page handle other vial sizes. Note that the mass on a PEG-MGF label may include the PEG chain, which can weigh more than the peptide itself, so the amount of peptide in a “2 mg” vial is uncertain unless the certificate says which is being weighed. These are calculations, not recommendations.

Forms and routes

The PEG-MGF peptide is sold as a freeze-dried powder for subcutaneous injection, usually in 2 mg vials, and is often marketed alongside unmodified MGF as a two-part protocol: MGF injected into the trained muscle after a workout and PEG-MGF injected on rest days. The distinction assumes that unmodified MGF acts locally and that PEG-MGF circulates, neither of which has been measured. Some listings describe oral or nasal PEG-MGF, for which no study exists in any species. PEG-MGF vs MGF is, in short, a comparison the published literature cannot make.

PEG chains come in many sizes, from a few hundred to tens of thousands of daltons, and can be attached at different points on a peptide. Vendors rarely specify either. Two products both labelled PEG-MGF may therefore contain quite different molecules, and the stability, clearance and activity claimed for one cannot be assumed for the other.

PEG-MGF side effects and safety

There are no human safety data for PEG-MGF and no animal toxicology. The concerns are inferred from three directions. From MGF: the E peptide increases IGF-1 receptor signalling [7], IGF-1 signalling promotes cell growth, and a 2026 review of growth hormone-axis peptides described mitogenic concerns about IGF-1 analogues, including PEG-MGF, as biologically plausible but unproven, while listing dysglycaemia, fluid retention, joint and muscle pain and injection-site reactions among adverse effects reported across the class [5]. From the mouse study: continuous exposure to the rodent E peptide made muscle larger but weaker [12], and PEGylation is designed to make exposure continuous.

From PEGylation itself: PEG is generally regarded as non-toxic and low in immunogenicity [4], but the pharmaceutical experience with PEGylated proteins comes from defined conjugates made under controlled conditions. A PEG-MGF vial from an unregulated supplier has no such characterisation, and a peptide that has never been made under pharmaceutical conditions cannot inherit the safety record of drugs that have. Black-market MGF products have already been found to contain modified analogues rather than the natural sequence [17] [18] [19], and there is no reason to expect PEG-MGF products to be better characterised.

A 2026 review of peptides sold in sports medicine found rigorous human safety data scarce across the class and warned of the potential for serious harm [20]. For PEG-MGF the data are not scarce but absent.

Regulatory status and WADA

PEG-MGF is not approved as a medicine anywhere and has never been in clinical development. In sport, WADA has considered MGF and its analogues prohibited since 2005 [18], and growth factors and related substances affecting muscle protein synthesis fall under section S2 of the 2026 Prohibited List, prohibited at all times [21]. A PEGylated analogue is covered in the same way as the parent. Anti-doping laboratories have published detection methods for MGF variants [17] [18] [19], and UK Anti-Doping applies the same list [22].

In the UK, PEG-MGF has no marketing authorisation, and regulation 46 of the Human Medicines Regulations 2012 makes it an offence to sell or supply an unauthorised medicinal product [23]. It is not a controlled drug under the Misuse of Drugs Act 1971, whose peptide hormone entries name somatropin and chorionic gonadotrophin but not IGF-1 variants [24]. Testing a PEGylated peptide is harder than testing a plain one. PEG chains are mixtures of lengths, so a mass spectrum shows a spread of masses rather than a single peak, and a standard HPLC purity figure says nothing about whether the peptide underneath is the human MGF sequence, an amidated analogue or something else. A useful certificate should identify the peptide sequence, state the nominal PEG size and say whether the labelled mass refers to peptide or conjugate.
